Share Your Story with a DAISY Nomination

Wayne HealthCare is proud to partner with the DAISY Foundation to honor and recognize our extraordinary nurses for the outstanding care they provide to our patients.

3-Step Nomination Process:

1. Pick up a nomination form from any DAISY award wall rack/collection box located throughout the hospital or access the online form. 
2. Complete the form and describe at least one specific situation or story that clearly demonstrates how this nurse made a meaningful difference in the care of a patient. Anyone, including patients, family members, hospital staff, visitors, and volunteers may nominate a deserving nurse.   
3. Once the form is completed, if you didn't use the online form, simply drop your nomination into any DAISY collection box or mail it to the address below. You can also email your story to Kim.Freeman@waynehealthcare.org.

The DAISY Award was established by the DAISY Foundation in memory of J. Patrick Barnes who died at 33 of ITP, an auto-immune disease. The Barnes Family was so impressed by the clinical skills, caring and compassion of the nurses who cared for Patrick that they created this international award to say “thank you” to nurses everywhere. The DAISY Award is the highest honor a nurse can receive at Wayne HealthCare and is awarded twice a year.
