Community Health Needs Assessment (CHNA)
Wayne HealthCare has a direct initiative to assess and identify the primary health needs of people living in our community. The Community Health Needs Assessment (CHNA) is an assessment of our community’s health status, hurdles to care, and other demographic and social issues affecting our community. The assessment will help focus resources and drive change to support healthcare prevention.
